Still waiting on a response from Minhea, PFlow is a much better workflow for the things I do.

I'm not sure why it doesn't work in your case. I've set up a simple scene containing a teapot, a proxy of the teapot and a PFlow system which creates instances of the proxy:

Have you tried using a proxy of a MXS reference?

yes. they work.
here's some tests i did a few weeks ago using pflow to scatter proxies.
it's set up just like mihnea's example above except that it uses a "position object" instead of "position icon" and remains static.

It obviously works, so I'm stumped. The un-proxied original objects and the MxProxy objects render but the PFlow ticks just turn into bounding boxes. I notice that the teapot example shows a point cloud representing the MxProxy, I just get a bounding box.

Minhea, I sent you a MAX file. Please take a look.

Not sure how to use a proxy of a MXS reference.

We have found an issue with PFlow instances and motion blur, thanks for the scene. Until we release a fix, if you don't need motion blur, turn it off in the render globals and the proxies should render fine.

In the scene we received from you, motion blur was set to Motion and Deformation. After we switched it to Off, everything started to work. We tested it in Max 2012. Did you make some modifications in the scene after you send it to us? In case you did, can you resend us the scene? bogdan.coroi@gmail.com or mihnea.balta@gmail.com

? Maybe I'm looking in the wrong place. The options I have in the General Settings tab for motion blur are: per object, on, off. I don't see anything about deformation. I have MB set to off.

Hmm, it seems you have an older version of the plugin. Check Customize->Preferences->Maxwell tab and see what version you have. The latest should be 2.6.18.
